This Case Management and Care Coordination for Primary Care Nursing Micro-credential program will assist nurses to understand and develop skills in case management and care coordination in the context of primary care nursing, including chronic disease management, health behaviour change, and addressing complex social determinants of health. Learners will use case management and care coordination principles to inform comprehensive care plans for clients, in collaboration with the interprofessional team. This micro-credential is part of a series of stackable micro-credentials for primary care nurses
